The purpose of this data dashboard is to support client tracking and reporting processes for providers and project leadership. The dashboard provides current information from SAMHSA’s Unified Performance Reporting Tool (SUPRT) and a record of the Government Performance Reporting Act (GPRA) data collected prior to October 2025.

Client Status tells you if client is currently enrolled or discharged.

Next Assessment Due tells you which SUPRT is next due for the client.

Next Assessment Sections Due tells you which SUPRT sections are due for that client at that assessment time point (A and/or C version).

Next Assessment Open Date and Close Date shows you the window of time to complete the SUPRT for each assessment time point.

6-month Status and 12-month Status tell you the completion status of assessments at these time points.

Click to View All Data

Some tables are too large to show all columns and rows within the main dashboard screen. Click on any table name that contains “click to view all data” to see and scroll through all rows and columns in the table. Clicking on the table name will bring you to a new dashboard page displaying only the full table selected. Use the scroll function at the bottom and right side of your screen to scroll through the table and view all columns and rows, as needed. To return to the dashboard tab you were on before expanding the table, click the back button on your browser or the original page name (highlighted in blue under your organization’s logo). 

Download Data Tables and Visualizations

When you hover over a table or other visualization, a button with three dots will appear in the top right corner of the tile. When you click this button you will be presented with the option to download the data selected in an Excel or CSV file, or to download the data visualization in a PDF or image file.

Filter Data

The filter function (centered at the top of each tab of the dashboard) can be used to narrow results by a specific site, provider, client, and/or time range. First, click the type of filter you want to apply, then select the filter details. Once you hit “Apply” the system runs the filter and the dashboard will update the data for relevant tiles. To turn off the filter, you must select “Revert to defaults” or turn off the filter setting you selected, then hit “Apply” again. You may apply multiple filters at the same time.

Hover Over Data to View or Highlight Values

When you place your mouse over data visualizations, hover boxes appear to highlight the particular data point(s) selected and offer other relevant information (if applicable). Some label text may be too long to display fully on the dashboard; hover over these labels to display the full text.

If needed to contextualize the data presented, a Notes section is included at the bottom of the dashboard tab. Notes pertaining to a particular dataset or visualization are marked with asterisks.

Show or Hide Data in Charts With Legends

Clicking on an item in the legend of the chart toggles the inclusion of the corresponding “slice” in the chart. When a slice is excluded its value is removed from the aggregated total and the sizes of the remaining slices are adjusted accordingly.

Sort Table Data

If you want to sort the information in a table, click the column title to rearrange the data from descending to ascending order (an arrow will appear to indicate the order). You may find it helpful to sort table data by site and/or provider name, assessment status, or the date next assessment form opens.


  • SAMHSA requires Baseline and Record Closeout SUPRTs to be completed for 100% of clients receiving grant-funded services. Refer to the Client Tracking tab to monitor SUPRT completion rates.
  • Dashboard data are updated daily. If data do not appear to be current, clear your browser’s web history.

Multiple layers of security protect the privacy and confidentiality of participants, and PRG staff receive training in the proper management and use of confidential data. Participant dashboard data are de-identified or aggregated, and access is restricted to authorized users only. These data are to be used solely for approved programmatic, evaluation, or reporting purposes. Please report any suspected data inaccuracies, privacy concerns, or technical issues to PRG.
